1209參訪紀錄

學期過了一半，終於有機會實際參訪機構了，學姊事先寄來住民的藥歷檔，希望我們先看看有無問題，又鄭重其事的要求我們穿上藥師服，說是要與醫療團隊開會，使我的心情既是興奮又有些忐忑不安。

機構位於寧靜巷子裡，附近的居民似乎很習慣這條巷子來來往往的白衣人士。
是一棟四層樓的建築，外觀看起來十分整潔，屋裡透出的黃色燈光在這寒冷的天氣令人感到特別溫暖舒適。走進一瞧，一位住民坐在門口的椅子上抽菸，照護人員半開玩笑的問，是不是今天抽的第一支菸?

一群人安靜地來到第三層樓，各層的交誼廳都有住民在進行日間活動，
每層樓牆上都貼著作息表，這情景跟在紀錄片上看到的差不多。
復健師和醫生一一檢查各個住民的身體狀況，藥師則翻看用藥紀錄，與機構的護士對談，獲得更多住民用藥的資訊，這種例行的參訪大約一個月一次，其中裡頭的住民進進出出，有的住院，有的回家休養。

觀察了一下，這邊的住民幾乎都沒有辦法自理生活，大多數作在輪椅上，並插著鼻胃管，藥物必須靠管灌的方式給予，除交互作用外，須將更多藥品服用的注意事項納入考量，例如：能不能磨粉? 劑量? 等等。機構每位住民藥物的管理也十分重要。

接近三小時的參訪，及輪椅種類及使用方式介紹後，醫療人員與機構的照護人員及護士營養師等，針對每一位病人的各個問題做詳細的討論，不同領域的專業人員互相溝通，使住民能獲得更好的生活品質。我期許自己不久的將來也能這樣與人談論自己的專業領域。

這次實地參訪收穫不少，期待下一次護理之家行。

不知以前也在哪裡聽過，
但那印象卻不是Girls傳達出的那樣悲傷，
反倒比較像第二個版本表達出的感覺，
比較輕快。
